Apart from the fins, he does have the body shape of a small shark. Now I look closer, he has a small hole in his head where the middle eye would have been.
I tried out the other fish effects, and shark01 is the only large fish I found with two eyes. There are three other large fish:

There are smaller fish with two eyes, but their eyes are just black dots in their textures. (fish01, fish02, tea_aquarium)
I forgot if normal fishes exist in game as they are never seen in Sea. (I think in fish tank they exist? i forgot)
Overall would be awesome if there's ways to script the shark to inflict damage to player/npc with a script to simulate shark attacks lol!
There's also an effect for a school of fish, which I'm sure I've seen in gameplay, but I couldn't get it to load in a cutscene (or at least, I couldn't find it if it had).
This is "tea_aquarium". It contains two small Angel fish, a Starfish and the filter tube for the fish tank in Saints HQ, all in one effect.
"fish01" is a Goldfish swimming clockwise. "fish02" is the same fish swimming anti-clockwise.
I see. I did not pay much attention to the fish in game. Everyday we learn something new even when its old. lol

There was no animation code in the cutscene file that should have allowed this to happen.
It even happened exactly the same when I replayed the cutscene again. It only stopped happening after restarting the game.
