I've had this one too. I have a nagging suspicion it's something about the way it hides hairstyles when a hat is put on, and then doesn't un-obscure when it's removed by exiting the sim, but no proof.
It does however fix itself the next time you go back into the simulation, so it's not a persistent bug, just a very mild one.
