As saddening as it is, I came here looking for an eyepatch mod, too. There isn't one for the third (I normally just cover the eyes with glasses), in the Pirate's Booty DLC for IV, there is an eyepatch. There are several on the Workshop, if you don't like the look of the DLC one.
