Its not a simple process tbh. Since it requires some 3d software skills to make them work properly
I extract the meshes using ninja ripper, edit the model to fit the player and then import into the game using the SR4 SDK
The weapon clone tool is kinda busted, some items would be cloned properly but might give you problems once you try to replace them. Happened to me before. For the same reason i only clone the swat smg since then.
Try cloning that and port it as a new weapon instead of a costume, see if it...
Im honestly out of ideas then... I have used that template for my weapon mods and they all worked so far. Not sure if your cloned item its the one causing the issue.

EDIT: Try this, couldnt convert it but i attached the fbx ready and the maya file. Your values were all over the place, clip scale and position was wrong and same with the tags.
I have the feeling the weapon template its causing you problems. Try mine, it only has one bone but i fixed all the problems the templates had, just add your weapon, move the tags and scale the bone to 2.54 before exporting it
Your tags are messed up, move them to the center of the grid, they should be (0,0,0) in their x,y,z translate values to work correctly.
If they arent, freeze their values (coordinates box> freeze> all) after you placed them in the center of the grid. And once they are at 0,0,0, move them to...
Overall it works as intended... but yeah this is caused by a mix of several stuff i did wrong back then lol

-Textures are too dark, which gives a darker color in game
-The lights in this game are beyond awful, everything is too dark and brighter areas just...
